How to Share a Client Input Form

While the Hosted Intake Form is a great way to collect new leads from your website, social media, or referral sources, there may be situations where you want a client/contact to update an existing client with new information. To do this, you'll want to utilize the Client Input Form. Follow these steps to send one out:
1) Create a Hosted Intake Form
Note: Only Adminstrator users have the ability to create/edit Hosted Intake Forms.
To start the process, ensure you've created a Hosted Intake Form designed for this purpose. On Senior Place Premium, you can create multiple Hosted Intake Forms, so if you're already using one, just create another for use with the Client Input Form. This tutorial shows how to create/edit Hosted Intake Forms.
2) Navigate to the Client where you want to send the Client Input form
Within your Clients Module, navigate to the profile of the Client you want to send an Input form to.
3) Click Actions > Share Input Form
🚨 Before sending an Input Form, please keep the following behavior in mind. 🚨
- Forms will expire automatically after 21 days
- Forms have last write privilege. This means that if you update a field in the client profile and then the form recipient updates/changes that same field, their change will be the one that remains.
- To prevent this behavior from creating issues for your team, ensure that the hosted intake form you create in Step 1 doesn't include information that you don't want to risk being overwritten through this process.
- Forms can only be submitted once. After a recipient clicks Submit in their form, they will no longer be able to access it or make changes.
4) Send Input Form
At this point, all you need to do is customize the request to send it out!
- Choose the Form you want to use. In this example, we chose our form named Update Client Form. The form you choose will impact the information your contact will need to fill out.
- Determine the email recipients. Note that if you send the form to multiple recipients, it can only be submitted by one of them (whoever completes the form first).
- Craft your email subject and body. Be sure to include {{link}} in the body. When the email is sent, that text will convert into a clickable link taking recipients to the form.
- Attach a file (optional)
- Click Send.
5) Monitor Input Form Request
Once you've sent the request, you can monitor it by clicking Actions > Share Input Form (like you did in Step 3).
- The Previously sent forms section will only show up if you've sent at least one form.
- In this area, you can see which forms you've sent, and to whom they have been shared.
- The Expires column shows when the form will expire (if it isn't completed).
- The Status column shows whether or not the form is Active.
- If you want to Cancel an Active form, click the trashcan icon 🗑️.
